What Does this MRI Report Result Mean?

A lady diabetic patient aged 55 years. Her MRI report shows that L4 and L5 disk level r in diffuse bulge,annular tear, extension with inferioir migration fom right to left paracentral location causing spinal canel stenosis, thecal sac indentation, left lateral recess stenosis, left L5 root compression, right lateral recess fat effacement & bilateral foraminal protrusion causing bilateral neutralframinal fat effacement. Left facet joint arthropathy noted on L4L5 level. L2,L3 and L4 disc llevel causing thecal sac indentation. left ligamenium flavum hypertrophynoted in L4,L5 level. Disc dessicatin, osteohytes, Schmori s nodes and modic changes are described. I want o know whether surgery is the only option in this case? wat other alternatives are there for its treatment. Plzzzz advise.

Hello

MRI findings indicate degenerative changes in lumbar spine.
Degenerative changes are age related findings.It is due to chronic stress on annulus fibrosis.

At L4-5 level,there is diffuse bulge of intervertebral disc with annular tear and herniation of disc inferiorly which is causing central spinal canal narrowing and indentation over thecal sac.
There is also compression of left L5 nerve roots and bilateral narrowing of lateral neural foramen.
There is also facet(bony joint in between two vertebra) arthropathy at this level.

At L2-3 and L3-4 level, there is indentation over thecal sac.

There is also thickening of ligamentum flavum and formation of schmorl's nodes.
All these findings are consistent with degenerative spine disease.

Majority of degenerative disc disease patients are benefited by conservative treatment in the form of physiotherapy,analgesics and muscle relaxants.
Surgery should be the last option.

Treatment can be planned after consultation with neurologist.
